It's our goal to provide the ideal puppy for you. Some puppies like to cuddle, some have
so much energy that they can't sit still! Someone with an active outdoor lifestyle wouldn't
be happy with a 3 lb dog which needed to be packed around and constantly protected;
when a larger dog which thrived on playing and running would wear down someone who
had limited mobility. Please complete this application if interested in a puppy. We will
do everything we can to be sure you get the right puppy and that it integrates into your
home as smoothly as possible. We want both you and your puppy to have a long and
happy life together!

By checking this box, you are stating that the puppy is not for resell and that you will never place it in a shelter. If unable to care for it, you will allow us right of first refusal or allow us to assist in finding a home for it.

We guarantee your puppy to be free of any life threatening congenital defects for one year.
Your puppy has received preventive care, such as de-worming and vaccinations and will be
examined again by our Veterinarian prior to placement. You are required to have your puppy
examined by your Veterinarian within 72 hours of the puppy's arrival. This gives you peace of mind
and insures the puppy arrived safely and without injury. Failure to do so will void this guarantee.
If your veterinarian suspects any health risk undetected by ours, we must be notified within 24 hours
by you and receive a signed statement on the vet's letterhead, stating the date of the examination
and the health risk. All costs incurred for the puppy are the responsibility of the buyer. We will honor
our guarantee by replacing the puppy, repairing the health defect, or refunding the purchase price,
at our discretion. Puppies sold as pets will have limited registration (without breeding rights)
and with a spay/neuter contract.
